All-on-4 is a full-arch tooth replacement protocol developed by Nobel Biocare and Dr. Paulo Malรณ in the 1990s. Four titanium implants are placed strategically in the upper or lower jaw โ€” two vertical at the front, two angled at the back โ€” to support a full bridge of replacement teeth.

The angled rear implants are the trick. By tilting them, the protocol uses the available bone in a part of the jaw where bone density tends to remain even when teeth have been missing for years. This avoids the need for bone grafting in most cases, and reduces the total number of implants from the 6 to 8 a traditional full-arch case would require to just four.

The result is a fixed bridge of teeth that does not come out, does not move while eating, and looks like natural teeth.
