What is All-on-4

All-on-4 is a full-arch tooth replacement protocol developed by Nobel Biocare and Dr. Paulo Maló in the 1990s. Four titanium implants are placed strategically in the upper or lower jaw — two vertical at the front, two angled at the back — to support a full bridge of replacement teeth.

The angled rear implants are the trick. By tilting them, the protocol uses the available bone in a part of the jaw where bone density tends to remain even when teeth have been missing for years. This avoids the need for bone grafting in most cases, and reduces the total number of implants from the 6 to 8 a traditional full-arch case would require to just four.

The result is a fixed bridge of teeth that does not come out, does not move while eating, and looks like natural teeth. Patients walk out of the trip with a working set of teeth — typically a temporary bridge fitted same-day, replaced with the final bridge at the end of the same trip after gum healing.

Prices are per single arch (upper or lower). Both arches together — sometimes called "All-on-8" by patients but really two separate All-on-4 procedures — typically run US$12,500 to US$14,500 when done in the same trip. Acrylic provisional bridges are included; zirconia or titanium-supported permanent bridges are quoted separately.

The All-on-4 trip timeline

All-on-4 is typically done as a single 10-14 day trip rather than the two-trip protocol used for single implants. Here is the standard sequence:

Days 1-2

Arrival, consultation, treatment plan

Land in your chosen Vietnamese city, rest, hydrate. Day 2 is the consultation: panoramic X-ray, CBCT 3D scan, treatment plan walkthrough, written quote, materials confirmation.

Day 3

Surgery + immediate-load provisional bridge

Implant placement under local anaesthetic with IV sedation in some clinics. The four implants go in over 2-3 hours. A temporary acrylic bridge is fitted the same day so you walk out with working teeth.

Days 4-5

Post-op recovery

Light activity, soft foods, follow-up check at the clinic on day 5 to confirm initial healing and provisional fit.

Days 6-9

Healing window + impressions

Rest, light tourism. Day 8 or 9: detailed impressions taken for the permanent bridge. Dental lab fabricates over the next 4-5 days.

Days 10-14

Final bridge fitting

Try-in of the permanent zirconia or titanium-supported bridge. Bite adjustments. Final cementation. You leave Vietnam with the final restoration in place.

Some cases need a return trip 6 months later for a final permanent bridge upgrade. This is case-by-case and quoted upfront.

Common questions

Is All-on-4 done in one trip or two?

Most cases close in a single 10-14 day trip. Some patients elect a second 5-7 day trip 6 months later to upgrade the provisional bridge to a permanent zirconia bridge. The single-trip protocol is the default.

Can I eat normally with the provisional bridge?

Yes, but soft foods only for the first 4-6 weeks. The provisional bridge is functional but not designed for hard chewing. The permanent bridge fitted at the end of the trip handles full chewing force.

Both arches in the same trip?

Yes — sometimes called "All-on-8" by patients. Both arches together is a longer trip (12-16 days typical) with surgery on consecutive days or split across the first week. Pricing is roughly US$12,500-14,500 for both arches when combined.

What if the provisional bridge breaks?

It happens occasionally with hard chewing in the first weeks. Vietnamese partner clinics will repair or replace at no charge during your trip. After you return home, repairs require either flying back or coordination with a local dentist for an interim fix. The country-wide guarantee covers repair flights inside the 12-month window.

How long does All-on-4 last?

The implants typically last 20+ years. The bridge on top — provisional or permanent — has a shorter lifespan. Permanent zirconia bridges typically last 10-15 years before replacement. Acrylic-titanium bridges last 7-10 years. Bridge replacement does not require new implants in most cases.

Can I have All-on-4 if I have severe bone loss?

Sometimes yes, sometimes no. The angled posterior implants in the All-on-4 protocol use available bone better than vertical implants. Severe bone loss may require zygomatic implants (anchored in the cheekbone) or All-on-6 (six implants for more support). The CBCT scan during your consultation determines what is feasible for your specific case.

Same-day fixed teeth or removable while healing?

All Vietnamese partner clinics fit a fixed acrylic provisional bridge same-day as surgery. You walk out with fixed teeth — they do not come out at night. Some operators elsewhere do removable provisionals; that is not the SmileJet partner standard.
