What is All-on-4?

All-on-4 is a full-arch tooth replacement protocol developed by Nobel Biocare and Dr. Paulo Maló. Four titanium implants are placed in the upper or lower jaw (two vertical at the front, two angled at the back at 30-45 degrees) to support a fixed bridge of 10-14 replacement teeth.

The angled rear implants are the core engineering insight. By tilting them, the protocol reaches denser bone in a part of the jaw where bone tends to remain even when teeth have been missing for years. This eliminates the need for bone grafting in most cases and reduces the total implant count from the 6-8 required by a traditional full-arch plan.

The result is a fixed bridge that does not come out, does not move while eating, and looks like natural teeth. Patients leave surgery the same day with a working set of temporary teeth (the provisional bridge), then receive the final permanent bridge before they fly home.

All-on-4 vs. All-on-6: All-on-4 suits most patients. All-on-6 adds two more implants for patients with heavier chewing forces, bruxism, or wider arches. Your CBCT scan determines which protocol is appropriate for your bone density and jaw anatomy.

Days 1-2 Arrival and consultation

Land in your chosen city, rest, hydrate. Day 2: panoramic X-ray and CBCT 3D scan, full treatment plan walkthrough, written quote confirmed, implant brand and bridge material selected.

Day 3 Implant surgery + same-day teeth

All four implants placed under local anaesthetic (IV sedation available). Surgery takes 2-3 hours. A fixed acrylic provisional bridge is fitted the same day, so you walk out with functional teeth.

Days 4-6 Post-op recovery

Light activity only. Soft diet. Clinic follow-up on Day 5 to confirm initial healing and check provisional bridge fit. Anti-inflammatories and antibiotics as prescribed.

Days 7-9 Healing window and bridge prep

Rest and light tourism. Your gums stabilise. On Days 8-9: final impressions taken for the permanent bridge. The dental lab fabricates over the next 4-5 days.
