The procedure

What is All-on-6?

All-on-6 is a full-arch implant protocol that places six titanium implants in a single jaw to support a fixed bridge of replacement teeth. Unlike All-on-4 (which angles two of its four implants at 30 to 45 degrees to navigate around bone deficits), All-on-6 places all six implants vertically, relying on adequate bone volume across the full arch to achieve primary stability.

The additional two implants change the mechanical picture considerably. Chewing load is shared across six fixation points rather than four, lowering the force on any individual implant-bone interface. This matters for patients with heavy chewing forces, bruxism, or a broader jaw where four implants would leave the distal segments under-supported. It also matters in cases where a patient has sufficient bone to support six implants but not the ideal bone density profile that makes the All-on-4 angulated approach reliable.

The clinical outcome literature supports both protocols. Moraschini et al. (2015, IJOMS Vol 44(3) pp.377-388) reported a pooled 5-year survival rate of 94.6 per cent across full-arch implant studies. All-on-6 outcomes sit within that range when case selection is appropriate.

Who All-on-6 is typically indicated for: patients with sufficient bone volume who have a documented bruxism history; patients with heavier biting forces (larger jaw, denser musculature, history of fractured implant-supported prostheses); patients with a wider arch where six implants provide better posterior support coverage; and patients where the treating implantologist considers four angulated implants insufficient for long-term load distribution in their specific anatomy.

Pricing

All-on-6 prices across the SmileJet cities

Prices are per single arch (upper or lower). Both arches together typically cost US$15,300 to US$19,500 when done in the same trip. See clinic-level pricing in the section below.

City Price range (per arch, USD) Single-trip length
Ho Chi Minh City US$7,950 – US$9,650 10-14 days, single trip See Ho Chi Minh City →
Da Nang US$7,700 – US$9,200 10-14 days, single trip See Da Nang →
Hanoi US$7,825 – US$9,400 10-14 days, single trip See Hanoi →

Trip protocol

The All-on-6 trip timeline

All-on-6 follows the same single-trip structure as All-on-4. The additional two implants add marginal time to the surgery session but do not change the overall trip length.

Days 1-2 Arrival, consultation, treatment plan

Land in your chosen Vietnamese city, rest, and rehydrate after the flight. Day 2 is the consultation: panoramic X-ray, CBCT 3D scan, treatment plan walkthrough, written quote, and materials confirmation. Six-implant cases require careful 3D planning to confirm bone volume at all six sites.

Day 3 Surgery + same-day provisional bridge

Six titanium implants are placed under local anaesthetic with IV sedation available at most partner clinics. Surgery typically takes 2.5 to 3.5 hours for a single arch. A fixed acrylic provisional bridge is fitted the same day: you leave with working teeth.

Days 4-6 Post-op recovery

Soft foods, swelling management, rest. A day 5 follow-up check confirms initial healing and provisional bridge fit. Light activity and indoor tourism are generally comfortable from day 4 onward.

Days 7-9 Healing window + impressions

Rest and light tourism. Day 8 or 9: detailed impressions taken for the permanent bridge. The dental laboratory fabricates the bridge over the following 4 to 5 days.

Days 10-14 Final bridge fitting and departure

Try-in of the permanent zirconia or titanium-supported bridge. Bite adjustments across one or two sessions. Final cementation. You leave Vietnam with the definitive restoration in place.

A two-trip option is available for patients who prefer to receive a provisional bridge on the first trip, allow 6 months of healing, then return for a final permanent bridge upgrade. This approach is quoted upfront and costs more than the single-trip protocol; most patients opt for single-trip.

Who it suits

Why All-on-6 over All-on-4

All-on-4 is the more commonly performed procedure and is appropriate for the majority of full-arch cases. All-on-6 is the better choice for a specific subset of patients.

Bruxism and night grinding

Cyclic grinding forces exceed normal chewing loads by a significant margin. Six implants distribute that cyclic load more evenly than four. Most implantologists recommend All-on-6 (rather than All-on-4) for patients with a documented bruxism history, paired with a night guard after the healing phase.

Heavy chewers and dense musculature

Patients with larger jaws, denser musculature, or a history of cracking or fracturing natural teeth place higher-than-average loads on their dentition. The extra two implants in All-on-6 reduce peak stress at each fixture, which is particularly relevant over a 10 to 20 year horizon.

Wider arch architecture

Some patients have a broader jaw where four implants, even with optimal angulation, leave the posterior segments with insufficient support coverage. Six vertically placed implants can span a wider arch more evenly, improving long-term bridge stability at the distal ends.

Insufficient bone for All-on-4 angulation

Paradoxically, some patients do not have the specific bone profile that makes All-on-4 angulation safe and reliable. If the ideal angulation site is deficient but the full arch has adequate bone overall, six vertical implants may be a better solution than attempting a four-implant case with suboptimal angulation.

The decision between All-on-4 and All-on-6 is made during the CBCT 3D scan and consultation on Day 2. Your implantologist will confirm which protocol is indicated for your specific anatomy before any treatment proceeds.

Cost and clinical comparison

All-on-6 vs All-on-4: cost and clinical differences

Price difference in Vietnam

All-on-6 costs US$1,200-2,400 more per arch than All-on-4 at equivalent clinics, reflecting the cost of two additional implants and the longer surgery time. In absolute terms: All-on-4 from US$6,500 per arch versus All-on-6 from US$7,700 per arch in Da Nang. Both represent 65-75% savings versus Australian specialist pricing. The additional cost of six implants over four is roughly the price of two standard single implants.

When the extra cost is justified

The additional cost of All-on-6 is worth paying when: the patient has documented bruxism with a history of cracking restorations; the jaw architecture is wider than average and posterior bridge coverage is a concern; the bone volume is adequate for six vertical implants but not positioned well for the All-on-4 angulated technique; or the implantologist determines that four implants are insufficient for the expected long-term load. These decisions are made at the CBCT consultation.

Same trip length as All-on-4

All-on-6 does not require a longer trip than All-on-4. The additional two implants add 30-45 minutes to the surgery session but the overall 10-14 day trip structure is identical. Day 3 surgery, Days 4-6 recovery, Days 7-9 impressions, Days 10-14 permanent bridge fitting. The same provisional-to-permanent bridge protocol applies whether you have four or six implants.

Bridge options are identical

The permanent bridge material options for All-on-6 are the same as All-on-4: zirconia (monolithic or layered), titanium-acrylic hybrid, or a staged provisional-to-zirconia approach over two trips. The six-implant footprint actually supports the zirconia bridge more evenly at the distal ends, which is one of the aesthetic and structural arguments for All-on-6 in wide-arch cases.

Materials

Materials and brand options

Implants

Straumann (Swiss, lifetime warranty), Nobel Biocare (Swedish, lifetime warranty), Osstem (Korean, 10-year warranty), Dentium (Korean, 10-year warranty). All four brands are stocked at top SmileJet partner clinics. Brand choice affects the total quoted price and is confirmed at consultation.

Provisional bridge

Acrylic with metal reinforcement, fitted same-day as surgery. Functional for soft foods during the healing phase. Stays in place for 4 to 6 months in the two-trip protocol; replaced with the permanent bridge at the end of the trip in the single-trip protocol.

Zirconia permanent bridge

Monolithic or layered zirconia is the premium bridge option. It is the hardest, most natural-looking material available, with no metal substructure and no risk of the acrylic chipping away from a titanium frame. Typical lifespan 10 to 15 years before replacement.

Titanium-acrylic permanent bridge

A titanium frame with acrylic teeth pressed over it. Lighter than full zirconia and easier to repair chairside if a tooth chips. Costs less than zirconia. Typical lifespan 7 to 10 years. A practical option for patients prioritising repairability over aesthetics.

Aftercare

Long-term care for your All-on-6 restoration

Daily cleaning protocol

An All-on-6 bridge sits fixed in the mouth and does not come out for cleaning. Daily maintenance requires: a soft-bristle toothbrush or electric toothbrush for the exposed surfaces; a water flosser (irrigator) to clean under the bridge between the gums and the bridge underside; and interdental brushes to reach the spaces around each implant abutment. Your partner clinic provides a specific tool kit and demonstration before departure.

Night guard for bruxism patients

If bruxism was one of the reasons your implantologist recommended All-on-6 over All-on-4, a custom night guard should be made within 4-6 weeks of returning home. Cyclic nocturnal grinding forces are the primary long-term risk to bridge integrity. A night guard reduces peak load on the bridge and implants during sleep. Most home-country dentists can fabricate a custom guard from an impression; cost is typically A$250-400 in Australia.

12-month home-dentist check-up

At 12 months post-surgery, schedule a check-up with your home dentist or a local implant specialist. Request a periapical X-ray of each implant site to confirm bone levels. Send the X-rays to your SmileJet partner clinic for comparison against the baseline CBCT taken at your Vietnam consultation. This check-up baseline is the most important long-term monitoring step and catches any minor bone level changes before they become significant.

Bridge replacement planning

Permanent All-on-6 bridges have a finite lifespan: zirconia bridges typically 10-15 years, titanium-acrylic bridges 7-10 years. Plan financially for one bridge replacement over the life of the implants. Bridge replacement in Vietnam remains far less expensive than equivalent work at home. Many patients return to the same Vietnamese partner clinic for their bridge replacement, often combining the trip with a return visit to a favourite city.

Patient FAQ

Common All-on-6 questions

What is the difference between All-on-4 and All-on-6?
All-on-4 uses four implants per arch, with the two posterior implants angled at 30 to 45 degrees to bypass bone deficits. All-on-6 uses six implants (all placed vertically), distributing chewing force across a wider footprint. All-on-6 is typically recommended for patients with sufficient bone volume who have heavier chewing forces, bruxism, or a wider arch requiring more support points.
How much does All-on-6 cost in Vietnam versus Australia?
All-on-6 in Vietnam ranges from US$7,700 per arch (Da Nang) to US$9,650 per arch (top-tier HCMC). Both arches together run US$15,000 to US$19,500. A Sydney specialist charges A$28,000 to A$38,000 per arch for the same procedure: the Vietnam saving is typically 65 to 75 per cent on the clinical fee, before flights and accommodation.
Can I have All-on-6 if I have been told I need bone grafting?
It depends on the severity of bone loss. All-on-6 requires sufficient bone volume at all six implant sites, more than All-on-4, which can work around bone deficits through angulation. If bone loss is severe, a bone graft may still be required before six vertical implants can be placed. In extreme cases, zygomatic implants (anchored in the cheekbone) may be the appropriate solution. The CBCT scan at consultation determines what is feasible for your specific anatomy.
Can both arches be done in the same trip?
Yes. Both upper and lower All-on-6 in the same trip (sometimes called "All-on-12" informally) is a longer procedure (12 to 16 days typical) with surgery on consecutive days or split across the first week. Both arches together typically run US$15,300 to US$19,200 when combined in a single trip, depending on city and clinic tier.
How long do All-on-6 implants last?
The implants themselves typically last 20 or more years. Moraschini et al. (2015, IJOMS Vol 44(3) pp.377-388) reported a pooled 5-year implant survival rate of 94.6 per cent across full-arch implant studies. The bridge on top has a shorter lifespan: zirconia bridges typically last 10 to 15 years; titanium-acrylic bridges last 7 to 10 years. Bridge replacement does not require removing the underlying implants in most cases.
What aftercare is required once I return home?
Standard aftercare: twice-daily brushing with a soft brush, daily use of a water flosser or interdental brushes to clean under the bridge, and a check-up with a local dentist at 3 months and 12 months post-surgery. If you have bruxism, a custom night guard should be made within the first few weeks of returning home. Your partner clinic will provide a full aftercare protocol in writing before you fly home.
Is the All-on-6 provisional bridge fixed or removable?
Fixed. All SmileJet partner clinics fit a fixed acrylic provisional bridge on the same day as surgery. You do not remove it at night. This is the standard of care at all SmileJet partner clinics; some lower-tier operators elsewhere provide removable provisionals, and that is not the approach used here.
