What is a wisdom tooth extraction?
Wisdom teeth (technically third molars) are the last four teeth to erupt, usually in the late teens or early twenties. Because modern human jaws have grown progressively shorter over thousands of years, there is often insufficient space for these teeth to emerge correctly. The result is impaction: the tooth is partially or fully blocked by the second molar, jawbone, or overlying gum tissue.
Impacted wisdom teeth cause problems in several ways. A partial eruption creates a pocket between the gum flap and the crown where bacteria accumulate, causing pericoronitis, an infection of the surrounding soft tissue characterised by pain, swelling, and difficulty opening the mouth. A cyst can form around the crown of an unerupted tooth, slowly destroying adjacent bone. Full bony impaction can put direct pressure on the roots of the adjacent second molar, causing resorption of that tooth's root structure.
Extraction is indicated when any of these pathologies are present or the imaging shows significant risk of future complications. It is not always necessary in younger patients with asymptomatic, fully bony-impacted teeth, but in patients in their 40s and older who have retained wisdom teeth, the risk of late-onset complications is higher because the bone has fully mineralised around the tooth, making late surgical extraction more involved than it would have been at age 25.

Whether your extraction is classified as simple or surgical depends on the CBCT scan result, not on symptoms. A tooth that has caused no pain can still be fully bony-impacted and require surgical removal.
IV sedation for extraction
IV sedation for wisdom tooth removal is available at most SmileJet partner clinics in HCMC, Da Nang, and Hanoi, and at selected clinics in Hoi An and Phu Quoc. It is not general anaesthesia (you remain conscious and responsive throughout) but the sedative produces a deep state of relaxation and retrograde amnesia that means most patients recall little or nothing of the procedure.
IV sedation is recommended in three situations: removing multiple impacted teeth in a single session (removing all four at once is common in dental tourism trips), significant dental anxiety that makes local-anaesthetic-only procedures distressing, and complex full bony impaction cases where the procedure will run longer than 45 minutes per tooth.
The sedative is administered through a cannula placed in the back of the hand. A dedicated sedationist monitors vital signs throughout. You will need to fast for 6 hours before the procedure and arrange an escort for the journey back to your accommodation: you cannot drive and should not be alone for the first 4 hours post-sedation. Most patients feel functional by the evening of the same day.

Third-molar pathology
It is a common misconception that wisdom teeth only need removal in early adulthood. Many patients in their 40s, 50s, and 60s carry retained wisdom teeth that were monitored and left in place for decades, and are now causing active problems.
Pericoronitis
The most common presentation. A partially erupted wisdom tooth leaves a flap of gum tissue over the crown that traps food and bacteria. Repeated infections cause swelling, pain, and in severe cases difficulty swallowing. Each episode of pericoronitis increases the inflammatory load on surrounding bone. Extraction eliminates the source permanently.
Cyst formation
A dentigerous cyst forms around the crown of an unerupted wisdom tooth and expands slowly through the jawbone. These are often asymptomatic for years and discovered on routine X-ray. Left untreated they cause progressive bone loss, weaken the jaw, and can involve adjacent structures. Treatment requires extraction of the tooth and cyst enucleation.
Adjacent tooth resorption
A horizontally impacted lower wisdom tooth exerts continuous pressure on the roots of the second molar. Over years this can erode the second molar root to the point where the second molar becomes unrestorable, meaning the patient loses two teeth instead of one. CBCT imaging identifies early resorption that plain X-ray can miss.
Crowding and bite changes
Whether wisdom teeth cause crowding of the front teeth is debated in the literature, but mesially impacted lower wisdom teeth can shift the second molar forward over time, affecting occlusion. Patients who have had orthodontic treatment are sometimes advised to remove wisdom teeth to protect alignment.
Combining with a dental trip
Wisdom tooth extraction is one of the most natural add-ons to a primary dental tourism trip. Because the extraction is a single-session procedure (same-day consultation and treatment in most cases) it fits into the early days of a trip scheduled around implants, veneers, or a full smile makeover.
The most common combination is extraction on day 1 or 2 of an implant trip. Removing a problematic wisdom tooth before placing implants in adjacent areas makes clinical sense: pericoronitis or cyst pressure near an implant site raises infection risk and can jeopardise osseointegration. Vietnamese oral surgeons will often recommend dealing with wisdom tooth pathology first and allowing two to four weeks before placing nearby implants, something easily managed within a longer trip.
Patients travelling for veneers or crowns sometimes find on their initial panoramic X-ray that a wisdom tooth is causing resorption of an adjacent molar that would otherwise need a crown. Extracting the wisdom tooth and allowing healing can stop the resorption and make the crown unnecessary. Recovery and flying home
Wisdom tooth extraction recovery in Vietnam follows the same clinical timeline as anywhere else. The extraction site is sutured (surgical cases) or left to clot (simple cases), and the first 48 hours are the most important for clot stability and early healing.
No rinsing, spitting forcefully, or using a straw for 24 hours. Soft, cool foods only: yoghurt, soup (not too hot), scrambled eggs. Avoid smoking. Apply ice packs to the face in 20-minute intervals to reduce swelling.
Swelling typically peaks at 48 hours post-op. This is normal. Switch from ice to warm compress on day 3. Continue soft foods. Gentle salt-water rinse from day 2 onward to keep the socket clean.
Swelling typically subsides significantly by day 5. Energy returns. Progress to softer normal foods. Attend follow-up check at the clinic if one is scheduled: stitches may be reviewed or removed.
Most clinics clear surgical extraction patients to fly 5–7 days after the procedure. Cabin pressure changes are not a significant risk once the socket is partially granulated; the main concern is dry socket risk during the first 5 days.
Dry socket
Dry socket (alveolar osteitis) occurs when the blood clot is dislodged or fails to form, exposing the underlying bone. It affects roughly 2–5% of surgical extractions and is more common in smokers and patients over 40. Symptoms are severe aching pain beginning 2–4 days post-op, often radiating to the ear. It is not dangerous but requires prompt treatment: the socket is irrigated and packed with a medicated dressing at the clinic. Do not fly with an untreated dry socket.
