What is All-on-4?

All-on-4 is a full-arch dental implant protocol that replaces an entire upper or lower row of teeth using only 4 implants per arch. The two posterior implants are angled to maximise contact with available bone, which often eliminates the need for sinus lifts or bone grafts that traditional implant cases require.

The result: a permanent, fixed bridge of 12 to 14 teeth supported entirely by the 4 implants. Unlike removable dentures, an All-on-4 bridge is screwed in. You brush and floss it like natural teeth.

The protocol was developed by Dr Paulo Malo in the 1990s and is now one of the most extensively documented full-arch solutions globally. Patients who would have previously needed 6 to 8 implants per arch can be treated with 4, which is faster, cheaper, and avoids most grafting.

All-on-4 is the right choice for patients with significant tooth loss, failing teeth across an arch, or existing dentures they want to replace with something fixed. It is not the right choice if you have only one or two teeth missing, in which case single implants are simpler.

Bali All-on-4 FAQ

How long does All-on-4 surgery take?

About 3 to 4 hours per arch under local anaesthesia plus IV sedation. You walk out the same day with a temporary bridge fitted. Most patients are eating soft food that evening.

Is the temporary bridge a real bridge?

Yes, it is a fully functioning fixed bridge. It is acrylic so it is lighter and easier to adjust during the healing phase. The final bridge in Trip 2 is the permanent zirconia or hybrid prosthesis.

Will I need bone grafting?

Most All-on-4 cases avoid grafting because the posterior implants are angled to use available bone. Around 1 in 5 patients still needs a graft, typically US$960 per arch at SmileJet partner clinics. We tell you from the CBCT scan in Trip 1.

Acrylic bridge vs zirconia bridge: which should I pick?

Acrylic-titanium hybrid is the entry option, lighter, easier to repair, around US$2,400 less per arch. Full zirconia is the premium option, more durable and more aesthetic, with a longer life expectancy. Most AU patients with an all-day chewing requirement opt for zirconia.

How long does All-on-4 last?

The implants themselves should last decades with good hygiene. The fixed bridge typically needs replacement at 10 to 15 years for acrylic, 15 to 25 years for zirconia. SmileJet's 12-month treatment guarantee covers the first year.

Can I get both arches done in one trip?

Yes, double-arch All-on-4 is offered at most SmileJet partner clinics. Both arches operated on the same day. Recovery is more demanding (2 to 3 days of hotel rest) but the total trip count is unchanged: 2 trips, around 14 days total.

What if my existing teeth need extracting?

Extractions are typically done same-day with the implant placement. The temporary bridge then sits over fresh extraction sites. This is a normal part of All-on-4 protocol and is included in the surgery price at SmileJet partner clinics.

How does All-on-4 compare to All-on-6 or All-on-8?

All-on-4 uses the minimum number of implants needed for a stable fixed bridge. All-on-6 and All-on-8 add posterior support, useful in cases of severe bone loss or when patients want extra mechanical reserve. Bali clinics offer all three protocols. Your CBCT scan determines which is right for you.
