You had 2 main complaints when coming to Sakura: toothache and the desire to replace the old filling that was no longer good. After examination, Dr. Sakura found that the old filling on the inside of the lower molar near the gum was rough and unstable, and the pain was due to periodontal bone loss in the furcation and distal root. Due to periodontal factors (pockets under the gum), the tooth is expected to last only about 5-10 more years (maximum). The problem of periodontal tooth sensitivity will not improve without root canal treatment to remove the tooth's sensation and also to avoid other complications that can occur at any time (not waiting for 5-10 years). 1.b. From determining the cause as above, Dr. Sakura explained to her why she should not fill a new filling on the inside as requested, because that solution will repeat the result of creating a bad filling like the old one (due to not being able to control saliva and related to the tooth root furcation area on the inside), and also does not solve the root cause of the problem of frequent tooth sensitivity. 1.c. Instead, the doctor proposed a solution for the periodontal sensitivity problem: root canal treatment (to eliminate the sensitivity symptoms, thanks to the tooth's loss of sensation) and crowning (to solve the problem of the old bad filling and is necessary after the root canal treatment). 2.
