In March 2022, we consulted Melike Hanım regarding bleeding and swelling in the front teeth of my 3-year-old son due to a fall. She advised him to start antibiotics for a week, and if this didn't resolve, he'd need root canal treatment under general anesthesia. A week later, she requested a photo. We were skeptical, so we visited another pediatric dentist the same day. On the contrary, this dentist said that children's dental problems would heal on their own, and that even if a root canal was necessary, there was no rush; healing should be allowed. Following the dentist's prediction, the swelling subsided quickly and he recovered on his own. I condemn this approach of diagnosing children based on photos and referring them directly to surgery, and I don't recommend it to anyone. Note: After your reply, I'm changing the word "implant" I accidentally used instead of "root canal treatment." However, this doesn't change the fact that the process is progressing as described, and that you've scheduled surgery for two days after the photo.

Hello, first of all, thank you for your message. I think there's been a misunderstanding, let me clarify. Dental implants are not performed during the primary dentition period, before children are fully grown and developed, which is around their twenties, and we never recommended this to you. You may have developed an abscess in your tooth after trauma; we base our treatment decisions on internationally published trauma guidelines. In cases of fractures involving the nerve of the tooth during primary dentition, and especially if an abscess forms after trauma, the trauma guidelines state that root canal treatment is necessary. This is because permanent teeth begin to form from the root tips of primary teeth. An infection at the root tip can cause the permanent tooth underneath to erupt with a structural defect called Turner hypoplasia. Therefore, it is recommended to remove the infected tissue or, if the root structure is suitable, perform root canal treatment. Since your child is very young and, despite a week of antibiotic use, the abscess is still visible as it was on the first day I saw you, I am legally obligated to recommend the most appropriate treatment for your tooth. However, I am making these treatment recommendations not based on my own opinions, but by following internationally accepted guidelines. If, after telling you to leave the abscess alone, your permanent tooth erupts with structural damage, I would not have provided legally correct guidance. However, whether or not to undergo treatment, or to accept the potential consequences, is entirely your decision. Based on my long experience in pediatric dentistry, I have done my best to help you. We took our 2.5-year-old son to see her today. She is a wonderful person with great care and attention. I was afraid she might traumatize our son with his first dental visit, but Ms. Melike examined his teeth patiently and lovingly, playing with him. She informed us after the examination.
