I do not recommend dental treatment by Dr. Aleksandra Barać at all. Incompetence, poor judgment, wrong choice of treatment method, and the result is a disaster in the attached. Update: after two years I can finally say that my facial asymmetry has improved. It was a Na-HYPOCHLORITE ACCIDENT that occurs when the solution exits the tooth canal through the root tip (apex) and reaches the surrounding tissues. This can happen due to: excessive pressure during irrigation, a needle that is too deep in the canal, an open or wide apex, root perforation, incorrect irrigation technique. PREVENTION of Na-hypochlorite (NaOCl) accidents in endodontics is based on the correct canal irrigation technique and solution pressure control. 1. Correct needle length The needle must not reach the apex (root tip). It is placed 1–2 mm shorter than the working length of the canal. This prevents the solution from escaping into the periapical tissues. 2. Use of special endodontic needles Thin and flexible needles with a side opening (side-vented needle) are used. Advantages: The solution comes out sideways, not directly towards the apex There is less pressure in the apical part of the canal 3. Slow and controlled irrigation !!! Sodium hypochlorite is injected: Slowly! Without strong pressure on the syringe! High pressure is the most common cause of accidents. !!! 4. The needle must be movable in the canal The needle must not be stuck in the canal. If it is stuck: The solution has nowhere to go The pressure increases It can pass through the apex into the tissue. Therefore, the needle should: Be free So that it can be easily moved up and down. 5. Patency of the canal (patency) Before irrigation, the canal is: Mechanically expanded with instruments Dentine debris is removed This allows for normal fluid return from the canal. 6. Rubber dam A rubber dam prevents: leakage of solution into the oral cavity swallowing or chemical injury to the mucous membrane. 📌 The most important rule in practice: needle short of the apex + slow irrigation without pressure. Reply to the owner: it was absolutely the dentist's responsibility, given that I felt the strong pressure of the disinfectant jet when rinsing the canal (and probably close to the upper limit of concentration considering the intensity of the reaction), and especially given the fact that at the moment when I started to react due to the extreme burning sensation, the doctor did not immediately stop rinsing but continued (!!!), which was crucial for the reaction to be so extreme. The nurse wondered why I was reacting even though I was under anesthesia, and the doctor grumpily commented "it can happen under anesthesia too". Then she tried to rinse out the hypochlorite with some other solution (which was logically impossible) and closed the canal!!! So she sent me home, and when I took a picture of the swelling from home and sent it, she called me back to come, opened the canal and gave me a corticosteroid injection. Thanks to the nurse, I found out the name of the disinfectant. It is especially unacceptable that the doctor could have assessed that the integrity of the apex was impaired based on the orthopan, because there was a change (cyst) on it and a very thin bone wall, so that under great pressure the hypochlorite could easily get into the soft tissue. Anyone interested in this topic can very easily check the facts and conclude whether such things happen by chance and unpredictably as you claim, or whether the responsibility lies to the greatest extent with the doctor performing the procedure. The feeling, reaction and consequences are as if Domestos (contains hypochlorite) was injected into your face. Not even in madness will it happen to you again.
