In August 2021, I had a third molar extracted. No X-rays or anything like that were taken before the extraction. Afterward, I complained of reduced sensation in part of my tongue (dental paresthesia) because a nerve was damaged during the extraction. Dear Martin, along with sending our greetings, the first thing we want to say is that we regret you are still experiencing lingual paresthesia. However, to be completely honest, I would like to remind you of a few points that are inaccurate in your comment: On August 27, 2021, we performed the extraction of your third molar (tooth 17) using a panoramic X-ray, which is on file in your medical record. You can request a copy if you wish. In that X-ray, the third molars appear impacted, but they do not affect the inferior alveolar nerve, which is the nerve that provides sensation to the lower teeth on your right side. During your surgery, in order to extract the tooth, it was necessary to slightly reduce the bone around the molar that was holding it in place. The paresthesia is in the tongue. The lingual nerve is not inside the bone; it runs through the soft tissue next to the tongue, and neither X-rays nor a CT scan can detect it. In the anatomical area where we operate, it's very unlikely that this nerve is present, but in your case, it apparently was. That's why, after the extraction, and despite our attempts to be as minimally invasive as possible, you experienced paresthesia. I want to emphasize that the affected nerve cannot be seen on X-rays or CT scans, and this is one of the risks that all patients may have during a lower third molar extraction. We've only had your case of persistent paresthesia over time. We have had transient paresthesia with one other patient, but that was due to inflammation and resolved on its own after weeks or months. That was precisely what we pointed out to you at your first check-up on September 3, 2021. We also told you that very few cases result in permanent paresthesia. You understood this at the time, as recorded in your case file. You were scheduled for a second check-up on September 23, 2021, which you did not attend. We knew you were leaving the country, and for that reason, we wanted to schedule this check-up before you left. While a check-up doesn't resolve the paresthesia, it does demonstrate our intention to support you through this unwanted effect, which is undesirable for both you and us, following the surgery. Since you left the country, we stayed in touch via Facebook through my personal account. The last message we sent you was on October 26, 2021, in which you mentioned you had inflammation and we recommended you get evaluated where you were staying. In that message, we asked you to contact me through Facebook with any questions or comments, as it's very difficult to provide a solution in person while you're in Europe. After that message, which I can also forward to you, there was no further communication, at least not that we've had from you. All this is to remind you that we have always tried, within our means, to support our patients when things don't go as planned. That's why I'm answering you as transparently as possible. I understand that it's unpleasant for you to have half your tongue feeling numb, and I'm truly sorry that this happened to you or anyone else, whether they are treated by us or not. I also understand your assessment and the perception you may have of the clinic. While we can't prevent this small percentage of unwanted effects from occurring—because, as in your case, they sometimes happen even when everything is done correctly—and it has more to do with the anatomy and position of some nerves that are in unusual locations and, I reiterate, cannot be seen on X-rays or scans, I want to make it clear that you were treated with X-rays, and we followed up until you stopped communicating with us. As a team and personally, we deeply regret your experience, and we explain to our patients that there is always a small percentage risk of paresthesia, especially lingual paresthesia, after a tooth extraction. I was looking for a dentist because of severe pain and found this excellent clinic. From the beginning, the person who scheduled my appointment explained everything clearly, and the service was outstanding, both from the assistants and the dentists. 100% recommended. They also have an automatic insurance system; I only had to pay the co-payment for my treatment.
